I AM OFFICIALLY DISABLED ;)

i am now officially disabled

and for almost 3 years na alam ko na pwedeng

mag avail ng PWD Card

di talaga ako nag apply

but when my doctor gave me a form

and told me to just fill it up and they will take care of the rest

i just did

and after a week

viola!

i am now officially disabled

PWD CARDS

NATIONAL COUNCIL ON DISABILITY AFFAIRS (NCDA)

The National Council on Disability Affairs is located in Don Antonio Heights in Quezon City. It was actually accidental that I posted an HIV-blog in one of the facebook groups and a long time friend (who happens to hold a position at NCDA) messaged me to come and process a PWD card.

I was uncomfortable about the thought of processing one, but he was able to explain to me what I needed to know.


THINGS YOU NEED TO KNOW.

The card classification is PSYCHOSOCIAL. This is the same classification being used for Cancer and those who are undergoing (or have finished) drug rehabilitation.

You do not have to explain to any merchant what PSYCHOSOCIAL is.


HOW TO GET ONE FROM NCDA

For the time being, REDx is able to process the PWD cards, simply send the PWD form, your confirmatory and your I.D. picture.

The PWD form can be downloaded at PWD APPLICATION FORM

You can get your confirmatory results copy from your treatment hub, or as an alternate, you may ask your doctor for an abstract.

1. Who do we contact?

In Baguio, you can directly go to your hub and the staff have already been briefed of the protocol and they know where to send this.

2. What are the benefits of the card?

To mention a few:

>> 20% off restaurant bills

>> 20% off medicines (prescription under the name of the PWD cardholder and PWD booklet needed, supplements and multivitamins may not be covered)

>> 5% off basic commodities (SM Supermarket and the likes at a ceiling of P1,300/week, PWD PURCHASE BOOKLET may be required)

>> Express lane at retail establishments, MRT/LRT & Banks

3. How long is the processing of the card?

The actual processing should not be more than a couple of hours once the application & attachments are received. However, REDx brings the application forms on a per batch basis and as such, it may take time depending on the bulk of the application forms collated by REDx

4. What if I get declined from using my PWD Card?

5. I had a previous bad experience in processing a PWD Card at my City hall, where can I report this?

SO YOU DON’T THINK YOU SHOULD HAVE A PWD CARD

This is not a requirement for the HIV Community. No one is being forced to apply or get the card.


LIMITED PROCESSING TIME ONLY

REDx can and will only process the cards until the end of 2016. Effective 2017, PWD cards will have to be processed at the local government unit level.

FIRST STEP

dami kong to-do list, kelangang magsipag, price to pay to stay healthy in this oh so positive world.  pumunta ako kaninang umaga sa pinakamalapit na philhealth office, konti lang naman ang pila, siguro mga 30 minuto lang akong naghintay sa upuan.


pinareactivate ko lang ang account ko, mula kasi nag freelance self employed na ako eh hindi na ako nakapag contribute sa philhealth ko. pinag fill up lang ako ng form at pinagbayad ng Php 300.00 sa cashier.

Php 300 peso contribution para sa 1 quarter (3 months). mura lang din pala parang 100 per month lang, not bad at all.  sabi nung girl activated na ulit ako agad at okay na ang coverage ko after 1 quarter.  pero pag may surgery at iba pang malalaking procedure, after 3 quarters pa daw so bali 9 months. 


ibig sabihin pwede na akong magkasakit starting october, at pwede na akong maoperahan starting april next year 2012, hahaha - looking forward to it!

pero seriously, ito ang unang ibinilin sa akin ng mga friends ko, malaki daw ang pakinabang sa philhealth lalong lalo na sa mga hiv positive people.  bukod sa mga coverage sa karamihan halos ng procedures, mukhang philhealth din daw ang magtatake over sa funding ng HIV program ng pilipinas as soon as maubos ang intenational funding come 2012.  so sabi nila importanteng may coverage ka sa philhealth para maging patuloy ang ARV medications mo for free, if ever nag aARV ka na. 

so ayan binigyan na ako ng bagong card, kailangan ko na lang i laminate.


1 step forward to the right direction, next step... mag enrol sa gym --- parang ayoko na sa Fitness First, i was with them mga 4 years ago.  grabe dati pa lang talamak na ang cruising at hadahan noon, kamusta na kaya ngayon? ganun pa rin ba?  il probably enrol sa Golds Gym na lang, me mga contacts naman ako dun, baka makadiscount pa. promise this week i will start na, buti na lang di masyado toxic ang trabaho ko.
